Patient Information means any and all information or data regarding a patient including information relating to his or her identity, demographic profile, health status, medical history, medical records, test results, treatment or stay in a health establishment.

Appointment Cancellations

If you are unable to keep your appointment, please let us know. No charge will be made for cancellations provided at least 24 hours notice prior to your visit. Failed appointments and late cancellations may however incur a charge.

It is important that you notify us of any change in your personal detail so that we may keep our records accurate.

Emergencies

If you have an urgent dental problem please contact the practice and we will see you as soon as possible. Outside normal hours the answer phone will provide an emergency number to give you the best advice.

We take care to ensure that information about our patients is treated with the strictest confidence in accordance with our practice policy.

We hope that you are satisfied with your visit to Chartwell and would be happy to recommend us to others. If not, please let us know why by contacting our Practice Manager or filling out the Feedback form. The complaint will be dealt with according to our complaints policy.

Disabled Access

Our practice is located over 3 floors but not all the surgeries have disabled access. If you have any specific needs, please call us in advance so we can arrange to meet your needs.
